We are relatively new users of RE and are attempting to use the phonathon form to create a campaign worker form for volunteers to use in making personal solicitations. I have created a query, selected the fields available under phonathon forms and now want to export the whole thing into a form (which we are trying to create) with the tear off coupon and right hand side for comments.
I need to move the data around a little and clean up a field or two. OK, three or four.

If anyone else is doing this, what fomat do you use –csv for the data and word for the document? This seems most likely, but when we get into the word document we are having trouble pulling the data in.

Does anyone have a sample of what their word document template looks like?

I don't have a great deal of time to produce this and could us any suggestions.

I created my own phonathon form that was 8 in wide by 4 in high and it was on continuous paper. I exported my file as Blackbaud Report Writer Database (MDB). I built my form in Crystal Reports 8.5 and formatted it the way that we wanted. I did not have a tear off feature, but I put the Constituent ID as well as Name, address, city, state, zip, phone and last gift amount and date information on it.

We have created our own forms with a tear-off portion that is sent back to the donor. Our forms are 8.5 x11 - the tear off portion fits in a regular envelope along with a return envelope. I use a query with Export to a Word merge file. We have used these forms for several years and have been completely happy with them. This way, there are a large number of fields that are available that are not available thru Mail. Admittedly, I have not checked the upgrade to see if the number of fields has been expanded!
